i. Computerisation
of ESIC establishment is under process.
ii. Employers are being issued new 17
digit code which shall be used on Computerisation,
till then the existing number should
be used for all purpose.
iii. Employers have been requested through
letters & advertisements to submit
fresh declaration forms (Form-1) under
New 17 digit employer’s code for
all their employees for issuance of
Smart Identity Cards to Insured Persons
& family members.
iv. Employers are advised to send their
employees alongwith family members/dependants
at enrolment Camps for bio-metric finger
prints & photographs. Presently
the camps are going on in Beliaghata,
Central Calcutta & Ballygunge Branch
Offices. It is going to be start between
16-19 Nov. 2009 for Birlapur, Tollygunge,
Salt Lake & North Calcutta Branch
Offices. The employers attached to these
Branch Offices may collect D.F. from
respective Branch Office and submit
it to Branch Office before the Camp
starts. v. Employers can also
have enrolment camps in their premises
for their employees if they ensure
100 IPs family unit enrolment per
day. For this they can request the
Branch Manager concerned.
